  • Travel arrangements


    Invited participants should make their own travel plans and the Foundation will cover all reasonable travel-related expenses. To get the negotiated rate at the meeting hotel, please make your reservation before January 3, 2007. At the end of the trip, please complete the expense form and attach the original receipts. Also, please include your social security number and the address to which you would like the reimbursement check mailed. Mail the completed form to:
